Notice of Confidentiality Practices
(Effective as of April 1, 2022)
As an elder care professional and practitioner, I maintain the highest standards of conduct in my service to individuals, family members, caregivers, healthcare staff, and other involved professionals.
I make every possible effort to keep personal information private as I uphold my duty to safeguard your protected health information (PHI). In general the law protects the privacy of my communications with the client. In most situations, I can only release information about client communications if written permission is provided. In the event that I would consult with another professional colleague in order to provide the best possible service to the client, no identifying information such as client’s name would be revealed.
Whereas the following circumstances are unusual in my practice, please be advised of these exceptions which could require reporting to the appropriate entity or State Agency:
If there is the possibility of immediate harm to client or someone known to client
If client threatens harm to self or poses a risk to self or others
Government agency request for information related to heath oversight activities
If it is made known to me that an elderly or disabled adult has been abused, neglected or exploited
Note that if any of the above circumstances would occur, I will make every effort to fully discuss the circumstances with you or your representative before taking any action. I will also limit my disclosures to only what is necessary by law.
